An ovary is usually connected to the uterus by an ovarian ligament. Each ovary hangs out like an arm or appendages together with the fallopian tube on each side of the uterus. The way the ovaries connect to the uterus allows it to move freely, and so twists in this connection can happen.
Sometimes the ovary can twist alone or together with the fallopian tube. This twisting can be promoted by an enlargement of the structure due to a cyst or a mass in the ovary or the fallopian tube. It used to be that the surgeon would just remove the entire twisted appendages; it was believed that the ovary had been strangled of its blood supply and would be no further use.
